Membership Renewal

Members are encouraged to submit their membership renewal forms together with the proof of payments before the 31 Mar every year or before MAFM holds its Annual General Meeting. Our bank account details are provided in the forms below.

To renew your membership, please follow the instructions below:-

2. Please take note that some of the particulars requested are repeated in the renewal forms to enable MAFM to continuously update our members’ information.

3. Submit the form together with the proof of payment of your renewal fees.

4. The Secretariat will issue an official receipt and the Membership Certificate.
